The participant is responsible for costs related to, but not limited to travel, lodging, meals, etc.

A CERTIFICATE OF COMPLETION will be awarded for Continuing Education Hours Certification. Additional paperwork requests must be submitted in writing to Micah Dobush.

CANCELLATIONS:
All cancellations must be in writing and sent by regular post or email to the Eyes of Source Office. If a cancellation is made less than two (2) weeks prior to the workshop start date, no refund will be made.
If a cancellation is made more than two(2) weeks prior to the workshop start date, a refund will be made minus a nonrefundable deposit.
